If you are unable to keep a scheduled appointment, please notify Starfish Services at least 24 hours in advance.  Late cancellations (less than 24 hours) or no-show appointments will be charged $100, which is not covered by any insurance.

Please note, I am only accepting clients age 16 and older. 

My schedule is open 90 days in advance.

After scheduling an appointment, you will receive an email containing the forms in an e-fillable/returnable format.  Please return the forms no later than 48 hours before your appointment to allow time for clinician review.  Thank you!
